Signs of ear pain in a 1-month-old baby?

A 1-month-old baby cannot directly tell us "my ear hurts," so signs of ear pain (otitis) appear more as general discomfort. The most common signs are:

• Discomfort and crying: Difficult-to-soothe crying episodes, especially when lying down or nursing.
• Difficulty nursing: The pressure in the ear during nursing can increase pain, so the baby may refuse to nurse or stop after a few sucks and cry.
• Fever: A fever of 38°C or higher may be present, but not always.
• Ear-related movements: The baby may pull, rub, or tug at the affected ear, or avoid turning their head toward it.
• Sleep disturbance: Pain increases when lying down, so the baby may not be able to fall asleep or wakes frequently.
• Ear discharge: If fluid builds up behind the eardrum and pressure causes perforation, yellowish or bloody fluid may drain from the ear; pain usually decreases somewhat.

A fever (≥38°C) in a 1-month-old baby always requires emergency evaluation — see a pediatrician the same day. Additionally, if the baby is very fussy, completely refuses to nurse, or has discharge from the ear, you should see a doctor right away. Do not use any ear drops or pain relievers at home; treatment is only given by your doctor after examination with appropriate antibiotics or drops.
