How to lower fever in a 4-month-old?

With your first baby, every health issue feels urgent, and I completely understand your worry. First, the most critical information: If your baby is under 3 months and has a fever of 38°C or higher, go to the ER without delay. Since your baby is 4 months old, the threshold is a bit more flexible, but we still need to be careful.

If fever is 38–38.5°C and your baby seems okay (eating well, playful, normal crying), try these first:

• Remove extra layers — too much clothing raises fever; lightweight clothing is enough
• Keep room temperature around 21–22°C, fresh air (not cold, just comfortable)
• Use a lukewarm (not hot, not cold) cloth to gently wipe the forehead, neck, and underarms

Never do this: apply vinegar or alcohol, cold showers, overdressing, or giving medication without a prescription.

When to go to the ER:

• Fever is 39°C or higher
• Your baby seems sleepy, unresponsive, or very fussy
• They're having trouble breathing
• Bluish color appears or they seize
• It lasts longer than 24 hours

Use fever-reducing medication (acetaminophen/paracetamol) only if your doctor prescribed it and only at the dose for your baby's weight — ask your doctor about the right dose for your baby's weight, not just the brand name.
