2-year-old persistent cough?

Persistent cough in a 2-year-old (lasting 2-3+ weeks) most commonly has these causes:

1. Postnasal drip (nasal discharge) – from allergy or sinusitis, worsens at night and early morning.

2. Allergic cough – seasonal, sensitive to dust/mites, can include sneezing or nasal itching.

3. Asthma/reactive airway disease – cough in episodes, triggered by laughing/running/exercise, often worse in cold weather.

4. Foreign body aspiration – sudden onset, choking sensation or stubborn solo cough with a specific history.

5. Cough after respiratory infection – most common cause: lingering cough 3-4 weeks after viral infection, often resolves on its own.

6. Pertussis (whooping cough) – in unvaccinated or under-vaccinated children, episodic cough with possible vomiting/whooping sound during breathing.

Parent recommendations:

• Slightly elevate head of bed at night (helps postnasal drip).
• Adequate fluid intake, warm water/herbal tea (honey is fine for children over 1 year).
• Avoid cold/air-conditioned environments and irritants like perfume/smoke.
• If allergy indicators present, refer to pediatric allergist.

When to definitely see a pediatrician:

• Cough lasts over 3 weeks.
• Episodic cough, sleep-disrupting, vomiting-accompanied cough.
• Fever, weight loss, blue discoloration, fast breathing present.
