Why doesn't an 18-month-old boy talk?

An 18-month-old boy not speaking can have several common reasons. First, it's important to know: individual differences in language development are very normal. Some children say their first words at 12-13 months, while others don't speak until 18-20 months and then suddenly explode with language.

Most common situations:

• Late talker (typically developing): The child understands everything, communicates with gestures and pointing, follows simple instructions. Usually just needs time.
• If receptive language (understanding) is also delayed: The child doesn't turn when hearing their name, doesn't understand simple directions. This requires a different evaluation.
• Ear infections or hearing issues: Frequent middle ear infections can cause temporary hearing loss, delaying speech.
• Focus on big motor skills: While learning major motor skills like walking, some children "pause" language development for a few months.

Current pediatric guidelines suggest that by 18 months, at least 3-5 meaningful words (mama, daddy, water, milk) are expected. Having more is even better.

When to see a doctor:

• No words at all, or loss of words previously learned
• No eye contact, no shared attention (not pointing and looking at you)
• Doesn't seem to be hearing well (no response to sudden sounds)
