If baby spits up while sleeping, will she choke? How should I position her?

I completely understand this worry — it's one of the biggest fears parents have.

First, the most important thing to know: A healthy baby won't choke if she spits up while lying on her back. This is what the AAP and international pediatric experts clearly state. Your baby's airway anatomy has protective reflexes (cough and swallow) built in to prevent spit-up from entering the lungs. The real risk actually increases when baby is on her side or stomach — airway can get compressed.

Safe practices:

• Always put her on her back to sleep. This is the safest position; positioning on her side or stomach because of spit-up concerns actually increases choking risk.
• Keep her upright after feeding (15-20 minutes). Burping helps but don't force it.
• Keep the crib empty: No pillows, blankets, bumpers, or wedges on the mattress. Just sheets and baby.
• Don't elevate the crib head — elevated head crib mattresses aren't recommended; there's a sliding risk.
• Let baby manage spit-up: During spit-up, if she's not turning blue, not making sounds, or not appearing distressed, don't intervene — she'll clear it herself. Only step in if she can't breathe, turns blue, or goes unresponsive.
