Why does breast milk smell?

There can be several different reasons for breast milk to smell, and in most cases it's not something to worry about. Here are the most common reasons:

• Natural fatty acids in breast milk: Milk may have a slightly sour or metallic smell due to its fatty acids. Milk stored in the refrigerator for a while may take on a soapy or bitter smell due to oxidation. This doesn't mean the milk is spoiled; it can still be safely used.
• Equipment cleanliness: If the breast pump, storage containers, or bottles aren't cleaned well enough, soap, detergent, or bacterial residue may mix with the milk and cause odor.
• Mother's diet or medication: Garlic, onions, certain spices, or some medications can temporarily change milk's smell.
• High Lipase: In this rare condition, an enzyme (lipase) in milk is more active than normal, breaking down milk quickly and creating a sour, soapy, or metallic smell. This milk is usually safe for baby but some babies may refuse it.

When should you see a doctor?

• If milk's smell becomes sharp, rotten, or pus-like.
• If milk's color also changes (green, yellow, bloody).
• If your baby shows vomiting, diarrhea, extreme fussiness, or rash after drinking this milk.
• If you have signs of breast infection: pain, redness, fever, or sores on the nipple (could be mastitis).

In this case, save some milk and show it to your pediatrician, explaining the situation. However, if the smell is mild and constant, it's usually normal—especially colostrum in the first days can smell different too. If your baby is happy, feeding well, and healthy, you probably don't need to worry.
