Why is a breastfed baby constipated?

These worries with your first baby are very normal, I understand. Let me give you some reassuring information first: real constipation is rare in breastfed babies. Because breast milk is the easiest-to-digest food.

In babies, constipation is determined by the consistency of stool, not frequency. Some breastfed babies go 5–6 times a day, while others go once in 5–6 days — both are normal. As long as the stool is soft and paste-like, there's no problem.

What looks like constipation but is actually normal: The baby strains, their face turns red, they grunt — this is usually because they haven't learned to use their abdominal muscles yet. If the stool is soft, there's no issue.

Signs of real constipation:

• Stool is hard, dry, in clumps (like sheep droppings)
• Baby is noticeably uncomfortable and fussy
• Belly is bloated and feels hard to the touch

Possible causes: Changes in mother's diet (dairy, caffeine, spicy foods), starting solid food in the baby (6+ months), inadequate fluid intake, or rarely a health issue.

If your baby seems truly constipated, your first step should be consulting your pediatrician. At home you can try: bicycle movements, gently pushing legs toward the belly, or a warm bath — these help gas pass. Definitely don't give the baby water, juice, honey, or anything else (especially if under 6 months old).
