Why does a 1-month-old baby have loose stools?

Seeing loose, sometimes mucus-like or grainy stools in a 1-month-old is often normal. There are several common reasons:

1. Digestive system immaturity: A 1-month-old's intestinal flora is still developing. Enzymes are not at adequate levels, so stool may not reach full consistency before being passed.

2. Feeding type:

• Breastfed babies typically have yellow mustard-like, watery, and grainy stools. This is usually a healthy sign.
• Formula-fed babies have slightly darker, more paste-like stools.

3. Foremilk-hindmilk balance (breastfed): If baby switches breasts frequently without adequate time on one, they get more lactose-rich foremilk. This causes gas, fussiness, and loose, foamy, greenish stools. Typically "finish one breast, then switch" helps this.

4. Infection: Sudden increase in looseness, bright green and bad-smelling stools, accompanied by baby's fever, lethargy, or spitting up suggests infection (gastroenteritis). Stools may also be slimy or bloody.

5. Teething: Teething does not happen at 1 month, so this is not relevant at this age.

When to see a pediatrician:

• Stool contains obvious blood (not black but may appear) and mucus
• Baby has not had a wet diaper for 8 or more hours (mouth is dry, no tears)
• Fever of 38°C or higher (emergency at this age)
• Spitting up, fussiness, refusal to nurse accompany it

Generally, 1-month-old stool consistency is not like an adult's—loose stools are expected in breastfed babies. If baby is nursing well, gaining weight, and seems happy, loose stools are usually not a problem.
