Why does a 1-month-old baby spit up after feeding?

Spitting up after formula is quite common in 1-month-old babies. There are several basic physiological reasons:

1. Very small stomach capacity: A 1-month-old's stomach is about the size of a walnut (30-60 ml). Large amounts of formula overfill the stomach and it overflows.

2. Immature stomach opening (lower esophageal sphincter): The muscle between the stomach and esophagus is not yet fully developed, so milk easily backs up.

3. Swallowing air: Babies swallow air while nursing; this takes up space in the stomach and comes back up with food.

4. Rapid feeding or excessive amounts: If formula is given too quickly or your baby is fed more than they need, the stomach cannot tolerate it.

Important distinction: *Regurgitation* (milk trickling from the corner of the mouth, not bothering baby, a normal "spill") is usually harmless. *Vomiting* is more forceful, projectile-like, and bothers baby.

When to see a pediatrician:

• Forceful, projectile vomiting
• Occurring after every feeding and weight gain is inadequate
• Bile-colored (green-yellow) or blood-stained spit-up
• Signs of discomfort, fever, or dehydration (dry mouth, low urine output)

Practical tip: Pausing 2-3 times during feeding to hold baby upright on your shoulder for gas relief, keeping baby upright for 15-20 minutes after feeding, and avoiding overly fast-flowing bottle nipples can reduce spitting up.
