Projectile vomiting in a 1-month-old baby (vomit that shoots forcefully from the mouth, sometimes traveling a meter away) is a condition that needs careful evaluation.

Not every episode of projectile vomiting is an emergency, but it's important to identify what's causing it:

Most common possible causes:

• Pyloric stenosis — most common between 2-8 weeks; the vomiting happens right after feeding, and the baby often wants to feed again immediately after throwing up. If this happens regularly (at most or every feeding), you should definitely see your pediatrician or go to the ER. Without treatment, it can lead to weight loss and dehydration.

• Overfeeding or poor positioning — occasional spit-up can happen; keeping the baby upright, burping after feeding can reduce this.

• Gastroesophageal reflux — usually appears more as spit-up from the corner of the mouth rather than projectile vomiting, though in some babies it can be more forceful.

Red flags requiring immediate medical attention:

• Green or yellowish (bilious) vomit, or vomit with blood
• Baby hasn't had a wet diaper for more than 8 hours
• Baby is restless, pulling legs up toward belly when crying (sign of pain)
• Soft spot on head appears sunken, dry mouth
• Fever (≥38°C)
• Unusual persistent crying or lethargy
